I start to believe Lily never had a proper medical examination before in her life. Medical stuff usually, if not always, is invasive on some level. The dentist will lean close and touch around in your mouth while you just lay and let it happen. If a doctor wants to examine your spine, you have to take off your bra. If you have stomach issues, they might shove a small tube and camera in your ass or down your mouth. No one likes stuff like that, and a good doctor knows it - therefore, talking and "feeling the vibes" is important, especially when we are talking about a surgery that is fucking big both for the physical and the emotional side.

I mean, again - the knee surgery is fucking nothing compared to the emotional intensity of a gender reassigning surgery, yet still, my doc was able to read the room and adjust; When the possibility of being awake during the surgery came up, I think the guy saw how suddenly distressed I was by the idea and once I declined and said I would much rather sleep over it, we started discussing the other options that fit my medical history - e.g: in my country, there are certain laws about a few harder/stronger medications, and about how often/between what times a doctor can give it to you.
In Lily's case, especially since dealing with such an emotional and intense topic, I can imagen that the doctor wanted to consult about past issues and again, get an idea about a few things (but again, I have no clue about surgeries like that and preparations regarding it just about the more general ones.)

Oh, I can totally see it happen. Again, I have no idea about the specifics of a gender reassigning surgery, but before my knee-bullshit, I was asked about my weight a few times - and even asked to step on the scale. Once during a consultation with the sleep-doctor, and once previous the surgery, since a few months passed. Yep! Definitely a totally sensible thing to allow literal children who still think broccoli is yucky to be able to refuse getting psychological care.

Regarding Lily's cavalier attitude toward her meds-- yeah, look, she's pumping herself full of a hormone that her body doesn't naturally produce, or at least not to the extreme levels that it's now being introduced into her system, and presumably taking suppressants for hormones she DOES naturally produce. That's going to fuck up your system. You might think you can tolerate it just fine because you have been for (however long), but you'd be shocked at the kind of shit your body can deal with until it just can't anymore.

When you're in a position like this, it can absolutely go sideways real fast if your pancreas or something decides that it's done putting up with your bullshit (I do not know if this would actually toxify your pancreas but it's the second funniest organ name I could think of; 'spleen' gets top billing). And if you aren't getting your levels checked regularly you won't have the warning signs to stop it before you wind up hospitalized. A blood test is really no big deal, either, unless you experience extreme hypotension following a blood draw. (Highly unlikely, and even if this is the case, a clinic usually has a bed you can lay down on if you're prone to pressure drops. Not an excuse.)

Speaking specifically about bottom surgery, I expect it's not just because of the hair growth (TMI), since she mentions being pestered about her institutionalization. Things might have changed since last I remember, but I believe in Canada, while you can technically pursue a medical transition through cosmetic surgeons, to get it paid for by the state you have to have a doctor's evaluation, and that's a lot harder to get approval for when you have psychiatric co-morbidities since your problem is likely something that can (and should) be resolved with therapy.

The fact that she needs after-market drugs instead of being able to go through a primary more or less confirms that even the Canadian system rejects the premise of her being trans and refuses to cover the expenses.
