Preface
At over 1700 pages at last check, the SRS Horrors thread has grown in not just length but notorious cases. Ones that have seared into brains and scarred others for life. Due to how large the thread has grown, however, it has become less accessible for newcomers to find these cases on their own, or to be easily linked to them by the old timers. Therefore, a "Hall of Shame" to form an index of a sample of certain cases was suggested.
Assume all spoilers have medical gore, nudity, or are generally NSFW.

Most of the following are cases infamous in-thread for reasons including severe medical malpractice or resulting in severe life of quality issues or fatalities. The remainder are noted studies or document leaks pertaining to the running of gender clinics or medical ethics in gender affirming care. While many of these results are extreme, they are reminders still that all of them can happen to anyone undergoing these operations.

Surgeon: Oren Ganor at Boston Children's Hospital
Perhaps the most extensively documented subject in the thread. FTM whose thread nickname originated from the octopus tattoo on the arm graft that became her neophallus. Initially gained notoriety after video reactor TT Exulansic discussed her TikTok videos where she detailed her surgical complications post phalloplasty. These included pulmonary embolisms, DVT in limbs, and infections in the kidneys and bladder. Eventually became completely unable to urinate from renal failure, which itself led to a kidney dialysis regimen. Initially blamed her situation on Long Covid until she finally admitted that it was indeed a result of her many phalloplasty surgeries. Gruffin passed on July 23, 2024 shortly after a brain bleed, allegedly from a stroke complication. She was 24.
Ironically, while Gruffin prided herself as a trans rights advocate (TRA), her death in that community was only acknowledged by a single friend that wrote a Reddit memorial post and a slapped together obituary (which is how Farmers discovered her passing a month after the fact). Meanwhile, Exulansic's followers and Farmers were much more attentive and reactive to it. Regardless of her polarizing reputation amongst the latter group, Gruffin stood to many as a symbol of the true cost of child transitioning.
18 year old MTF in Dutch Protocol Study (initial thread post, PDF mirror of case study in link)
Dutch MTF that passed away after e. coli exposure during the operation caused shock and organ failure. Debridement attempts resulted in his entire pelvic region resembling a crater when he died. Notably, his initial operation used his intestines to aid in constructing the neovagina due to his puberty blocking pre op preventing his genital region from sexually maturing.

Surgeon: Dr Sutin at PAI Clinic in Thailand
Australian MTF that committed suicide at age 20 less than a year after having SRS in Thailand. This was an unusual case where the subject in question was already deceased when first covered by the Farms. The "No Suggestion" username is for his mother Nicole Dean, who wrote on Reddit to blame his surgeon for his death. Her posts became increasingly unstable, such as writing about him as if he was still alive. Many consequently speculated if Bella was actually roleplaying as his mother, or if he existed at all. His existence and death were eventually confirmed after some sleuthing.

Canadian MTF that experienced a colorectal fistula in his neovagina soon after post-op. Has experienced several surgeries to correct it, likely for naught as he continued dilating it between surgical operations and complaining on Twitter about feces still coming out of it. Now has to wear a colostomy bag and has been consulted by doctors about Canada's MAID euthanasia program. Still recommends the clinic where he was butchered to people when he is not complaining on Twitter about how he was butchered there.
Is also a friend and follower of TRA Zinnia Jones, who congratulated him after the initial surgery and still sends the occasional emotional support.

Young MTF that underwent vaginoplasty by Dr Kathy Rumer, an infamously incompetent surgeon even among the transgender community, at Boston Children's Hospital. His incisions to the side of his neovagina experienced separation and became necrotic, resulting in three slits where his genitalia used to be. The complications were such that there were rumors of him passing away from surgical complications after a period of inactivity on his Reddit account. Resumed being inactive after user @Stan made contact and confirmed that he was not only still alive, but had also healed.

MTF that gained online notoriety not only for his punny birth name, but also for the surgical complications that he tweeted about. Experienced a colorectal fistula similar to Fistulissa, leaving him to regularly pass gas through the neovagina. Proceeded to become a nightmare patient to medical personnel for their lack of help despite his own lack of aftercare (including attempting to hookup with gay sugar daddies on Grindr), also just like Fistulissa.
"Landmine Hijra" (original thread post, with video)
Hijra (Indian transwoman/nullo) whose results resemble one's gnarled body parts post-landmine detonation, hence the nickname
Gay man in a full time BDSM master/slave pup-play lifestyle. His submission culminated in him undergoing a vaginoplasty for his master's pleasure. Has filmed himself being fisted in his neovagina by his partner, both with and without a puphood.

Internal-Fly was in the process of funding money to sue Kpaduwa for malpractice. It appears that it went nowhere after raising less than $50.

A young woman with cerebral palsy and epilepsy exploited by her parents Paul and Katherine LeRoy, who filmed her for sympathy, attention, and money on social media by claiming that she was also a trans man. Was given a double mastectomy in early 2025 despite requiring full time care and also enduring a stroke shortly before surgery.
Despite that many cerebral palsy patients have average intelligence, most depend on caregivers to make medical decisions for them due to the extent of their disabilities. There is debate if Mara should’ve had the surgery even if she gave her consent, given her circumstances. Cases such as Mara’s highlight the concern of medical ethics in trans healthcare. They call into question of not only who is eligible to consent for medical transition, but also how aftercare would be addressed.

Phalloplasty patient that catalogued her transition on TikTok. Originally intended to grow an artificial urethra in her arm for urethral lengthening in the hope of being able to urinate standing up. When the artificial urethra failed, the surgeons instead took skin grafts from inside her mouth. These left her unable to speak coherently, perhaps indefinitely. Current status unknown after privatizing her social media.
Surgeon: Dr Kaoutzanis (AKA "Dr K")
FTM whose initial phalloplasty was extremely long and girthy, wider than a soda can. Thread nickname originated from the neophallus's resulting appearance of a tree stump after her experiencing necrosis. While many FTMs seemingly regard their phalluses like newborn children, Stump Dick was notable in the degree she did so. Most notably, after describing its appearance on a doppler radar, she applauded its "strong heartbeat" despite everything it endured. Also wrote a post thanking her surgeon titled "Dr. K saved my phallus!," birthing a thread meme.
This case had a bittersweet turn of events. Multiple surgeries on her phallus intimidated her, leading to her to not pursue additional surgeries. A couple years after her initial phalloplasty she detransitioned, removed the remaining stump, and got married.

Abdominal phalloplasty patient whose stomach region was deformed into "patches" of flesh, prompting comparison to a patchwork quilt or a crab carapace. This is typical of Freet's work, judging by some of his other patients's results.

Surgeon: Dr Curtis Certulo in Boston
FTM that became a guinea pig for a form of abdominal phalloplasty that Dr Certulo, an established plastic surgeon, attempted to innovate. Her thread nickname originated with the gray cast securing her neophallus when she first posted pictures of it online, bringing to mind an elephant's trunk. Sans cast, the neophallus continued inviting comparisons from being curled at the tip despite countless revisions. After the surgery's failures became too obvious to ignore, Certulo ghosted all of his phalloplasty patients to resume his plastic surgery practice in Los Angeles. Elephant Dick decided to pursue a replacement surgery with another surgeon for reasons including this and general result dissatisfaction.

FTM whose neophallus's tip split open from necrosis, giving the appearance of a toothless lamprey mouth. Filmed herself walking around and pelvic thrusting to demonstrate its movement. Also claimed to have successfully had stealth intercourse with it. Despite these alleged accomplishments, she was dissatisfied with her results. Obtained a replacement neophallus that included a method of urethral lengthening where the phallus is split in half and left as such for months.
Surgeon: Dr Markovsky in Munich, Germany
German FTM whose erectile device eroded after she had sex with her phalloplasty. Later on, her neophallus somehow gained an extremely rare parasite that began eating away what the ED erosion did not destroy, gaining her the thread nickname "Parasite Dick." Eventually pursued getting a second phalloplasty as she desired to resume her OnlyFans career and to continue having "penile" sex with women.
Dutch FTM whose metoidioplasty resulted in her clitoris "rupturing." Continued with a phalloplasty that also resulted in complications.

Non-binary and nullo identified woman that paid Crane $10,000 out of pocket for all external sex organs and the vaginal canal to be excised, resulting in a smooth surface where only the urethra remains. Female nullification is illegal nationwide in the United States due to Female Genital Mutilation (FGM) laws. Crane therefore committed a federal felony by performing this operation.

While Jazz has his own thread with a solid OP, long story short is:
Considered by many as Patient Zero of the "Munchausen's by Trans Child" social contagion, or at least the most famous example. Jennings was diagnosed with gender dysphoria as a toddler under suspicious circumstances. After becoming the subject of a non fiction picture book, "I Am Jazz," and being featured on the ABC news program 20/20, he received his own reality show on TLC (also titled "I Am Jazz").
At age 17, he underwent an vaginoplasty performed by Doctors Jess Ting and Marci Bowers. Jennings's surgery was an experimental procedure that included stomach and colon lining to make up for his "insufficient material" (as a surgeon on the show described his genitalia) that was caused by puberty blocking. The neovagina fell apart within days of surgery, requiring him to undergo additional reconstructive surgeries in the following years. Bowers, who is also a transgender woman, has since admitted that Jennings's surgery was not only a failed experiment, but that the puberty blockers he was given starting at the age of 11 rendered him permanently unable to develop sexually.

Swedish MTF that helped turn Bhålaj, a stuffed shark from IKEA, into a transgender symbol after posting a photograph of himself hugging the toy in his hospital bed shortly after vaginoplasty on Reddit. Matsson was effectively homeless at the time, as he was living with friends, and also experienced surgical complications post op. Has since detransitioned.
"Locked in Pooner" (original thread post / Neurology abstract link (archive))
The case study of an "otherwise healthy" 23 year old FTM that had been on testosterone for roughly a year before developing a stroke while home alone. As she was not found for hours afterwards, she developed paralysis that progressed into a case of Locked in Syndrome, a condition where a person is completely aware of their surroundings and sensations and yet is virtually unable to move or respond due to full body paralysis. As many people afflicted with it never recover, her prognosis is extremely poor.

An affidavit written by Jamie Reed, a former case manager at a gender clinic affiliated with St Louis Children's Hospital in Missouri. Reed accounts how medical personnel operated at the clinic, including placing any child that came into their clinic on puberty blockers and/or HRT even when it was obvious to staff that the patient was being sexually abused, severely mentally ill, or a victim of social contagion. Among the patients described include a child placed on hormones despite claiming to be an attack helicopter, and an 18 year old that asked for her breasts to be reattached shortly after having top surgery at the hospital.
WPATH exposé (WPATH Medical Documents Leak thread)
A 242 page PDF compiled by investigative journalist Michael Shellenberger released in March 2024. These are files from within WPATH's internal database of surgeons, therapists, and other medical personnel specializing in transgender healthcare (and particularly those of children), most notably Marci Bowers. Many of these pages detail medical personnel discussing the negative effects of puberty blockers, HRT, and other forms of medical transition on often underage or developmentally disabled patients, from worsening mental illness to death.
Cass Report (document archive)
A report published in April 2024 concerning the findings made by Dr Hilary Cass for the United Kingdom's NHS regarding medical transition guidelines for transgender children. To the ire of TRAs everywhere, Cass concluded that medical transition, particularly puberty blockers, do not improve children's health and may actually worsen it. She also noted a sizeable spike in more girls being diagnosed with gender dysphoria in recent years versus boys, suggesting a trend in overdiagnosis. She therefore recommended that non medical transition such as therapy be prioritized. Shortly after this report was released, the NHS heavily restricted use of puberty blockers for transgender childcare, to the point that their use for this purpose was effectively banned in the UK.
