Okay, sperging here but:
1) We are only seeing the ones that hit the target, not the ones that get shot down along the way
2) Sticking your neck out to intercept the cruise missile is often what gets you killed by the anti-radiation missile.
3) In the case of Delilah, the missile can effectively act as both. That means, if you're an Iranian radar/SAM operator, and you see those blips on radar, even assuming you (or your boss/intel guy) is confident those are Delilah missiles, then you have no fucking clue whether or not they're coming for you or coming for a target in the airspace you're tasked to defend.

I will actually go out on a limb and say that part of the reason the Iranian AA has taken such a hit is precisely because they were likely trying to fight back.

Depends on how the reactor reacts to the explosion. Unlike on Chernobyl, this reactor doesn't seem to be for power generation, so it might act differently
Plus it's deep underground and not out in the open, if there's a leak radiation might not escape as easily
