I mean, they're both probably twins, same age, one of them is trans so they could've been born male identical twins, and like you said, same face. There is some logic here, just covered up in all the same annoying tumblr trappings as the others.
Snarky and Obvious observations are as plentiful here as the red noses and the down with Cis shirts. It's fine as it is but to me I don't like how doesn't add that much to the conversation. While there is definitely obvious problems I think it would be better to address the smaller stuff that people don't see at first glance.
Like for example I have a problem here with posing of these two, The trans chick is leaning into her brother's shoulder with one foot off the ground so realistically, she's leaning in pretty hard. At the same time though, the dude's body language looks like it's barely affecting him, in fact he's got his foot slighty raised like he's ready to walk or his foot is itching. Objectively these poses are fine on their own but together they create this slightly off image. It doesn't ruin the whole picture or chill vibe the artist was probably going for what with the soft colors and the casual Hawaiian shirts, but it is a legitimate problem to consider.
Sad thing is this is what I mainly don't like about tumblr art. Tumblr often blinds artists to their flaws and inflate their ego, making them think their art is perfect because as long it follows this pro-LGBTQ+ agenda it doesn't need to be anything else, it doesn't need to change or improve, and so the artist becomes stagnant and stubborn. Any critique at all is just believed to be critique from a hater or a Cishet jackass who oppresses and bullies the precious and perfect social justice ally artist. I hate to think this artist's work is going to look the same in 1, 5, or even 10 yeards from now, unchanging because a dozen danglehairs and two demi-boy tweens convinced this artist that s/he wouldn't need to change.
I actually like this piece of work sans the extra tumblr writing and to me this artist definitely has potential, but its a shame to know s/he probably ain't gonna listen to criticism and learn.

This is often why I think whenever someone will draw an anime character fat it's not because of a fat fetish but more likely projection. Fatties still want to be seen as beautiful even if they are insecure about their bodies.
