It takes many months to heal properly. You come across a lot of "why does my new snatch look like ground beef" posts on places like Susan's in the months after people have had surgery. It's very much still an open wound at the moment, which means it will be quite a while before Phil reaches the stage where complications are unlikely.

I wonder how he's keeping up with the maintenance and dilating... I'm kinda disturbed it looks like there was no attempt to create a look of the labia minora.

You'd expect that if Phil went for the cosmetic option, but there'd be no need for him to dilate if that's the case. There's also no way Phil would admit it if adjustments to the plan had to be made during his surgery.
