The question I have though is why would this Dr Duggi character risk his professional standing by doing a pickle inversion on someone who clearly is not of sound mind and has not gone through any of the other bits that are required to transition or suchlike? I think the issue is that a lot of what doctors know about a patient is what the patient tells them. There’s no reason for Dr Dugi to look at Phil’s Tumblr and even Phil is probably not dumb enough to start pretending Xochi’s talking to him in the doctor’s office. If Phil answers the questions right, and there’s no contraindication on file, they have to take him at his word.

I think what gets to me the most based on the picture is the fact that he obviously doesn't shave down there or puts something to sanitize the wound in. Like most surgeries of that kind would suggest to use sterilized tampoons, of course washing it out (which depending on the depth may hurt like hell) and so on.
