Honestly I don't think either of them are "Hideous" maybe a bit homely looking. Holly imo only looks worse because her eyebrows contrast so sharply with her candy-colored hair. I wager she looks a lot better with her natural color.

I think Heidi looks fine without makeup. It's always going to look jarring seeing someone with 0 make up right next to them with a full face on, but she looks pretty without it too imo.

If it was a ploy, USCIS might want to take a look at it.
Probably it was real at some point, though, since there is a huge amount of paperwork going into every application for citizenship, and for ones that are based on marriage, people provide various kinds of proofs - joint accounts, mortgages, birth certificates of children, tickets to vacations, etc. US-spouse usually proves the income that can support the couple, as well.
Just imagine working on the application and hopping through the loops of regulation for 2-3 years to be dropped like this.

To put it bluntly Holly presumably puts out, fucks hard, eagerly sucks dick when she's on the rag instead of complaining about her cramps, etc, etc. For a short-sighted hypersexualised dunce those are desirable character traits and as long as she's not fat or a leper, will skew perceptions of her and make her seem like a desirable catch.
