This is actually pretty rare in my admittedly limited experience. Most of the time the low intelligence, other problems relating to the trisomy, or both, cause people with Down syndrome to be fat. Like, really fat. When you combine that with the ones whose mouths hang open and tongue sticks out 24/7 it gives the impression that they have no neck.

When I was in high school occasionally you'd see the higher functioning sped class going to/from their segregated PE class. One of the sped kids obviously had Down syndrome and it legit looked like he had no neck from most angles. When some of my classmates and I hadn't seen him for a while we wondered if he died. One day we walked right by the group and one of my classmates took the opportunity to ask the teacher/tard wrangler if "the kid with no neck" had died.
