I really feel like people like this are simply being failed by our education system. Just taking this person aside for 5 minutes for a calm one-on-one about simple definitions of terms would probably clear things up for this person a *lot*.

1. All girls have breasts and a vagina. Breasts can be surgically removed for certain purposes (i.e. mastectomy to treat breast cancer), but all girls are born with them and a vagina. There are no exceptions to this.
2. You are not a lesbian trans girl who isn't attracted to women. You are a gay man who is attracted to effeminate men. This is actually just fine, and totally okay, and also far less complicated for you to learn to cope with.
3. If you disagree with either of the above facts, it's because you're attention- or clout-seeking, not because you're genuinely confused. This can be corrected with therapy or mockery, your choice.

Society is just failing these people hard in this regard. Even if you call our past corrective measures "bullying," it worked to help correct these aberrations and misunderstandings.

What makes someone a 'woman' is literally your Chromosomes, your DNA. You are female. You are XX. No amount of surgery, hormones, or other medical intervention can change this. You can dig up a skeleton and run it's DNA and it will tell you, immutably, that this person was female. It still amazes me how angry some of them get when you bring up the genetics of it. The very same crowd that screams "THE SCIENCE IS SETTLED!" suddenly wants to ignore the pretty firmly settled science concerning sex. Cracks me up every time.

This is partially the fault of social sciences departments getting uppity. They're so entrenched in their beliefs and their thought models are so all-encompassing and completely impossible to falsify, they think they have a perfect understanding of the world and so biology's effect on the human mind and body can be ignored entirely.

So, if biology (or basic physical reality) doesn't matter and social constructs and factors define the human condition, it makes perfect sense for them that a 300lb balding man who claims to be a 10-year old girl in pigtails and a frilly dress for all intents and purposes is a 10-year old girl in pigtails and a frilly dress.
