I think Tom's recent problems, like OPL's stem from a failure to realize that he is a hideous dog mongler, and baby raper, who has been shunned by God himself.
 
As a former psychiatric nurse assistant and a current cop, Tommy is most likely going to get out after less than a week in a temporary ward and forced on medication with welfare checks to the shit hovel. He is just a nuisance at this point, if he commits crimes then he'll be back for longer stays. Most states have shitty mental healthcare because of past federal grant cuts. They try not to commit crazy people that actually deserve it for the sake of society, unless they commit crimes. Kinda defeats the purpose of preventing crazy people from being a bane on society at our expense, but it does offer more chances for mentally ill people to get on medication and potentially live normally.

Sadly, mental healtcare is so cucked it takes another patient to have to complain for the doctors to actually put in an order for a forced shower. It's usually not prison style hatred for chomos in mental hospitals, either. There are patients who care about it and hate them, but it all depends if they find out and are not crazy/doped up to. In the event that a patient hates another patient they usually move into separate pods/wards.

I used to be an eccentric alcoholic, and was picked up for a psych eval, then committed for further observation on suspicion of being Psychotic on the words of a particular psych that later was stripped of her license for ethical violations. I was distraught at first, but learned ofthe many perks of being in the Psych ward. Free rent, free power, free hot water, free meds (Just had to ask.) Free food, and free smokes. I adapted well, earned the staff's trust, then used my sign out powers to buy spices and sauces for the communal meals, and used this time out of my mother's basement to look for an apartment via the yellow pages and secure my independance. Later it was determined I was just a weird guy, weird, but sane and highly intelligent.

I saw a lot in there. Weird as fuck trannies that openly cut themselves at the food table and had to be watched closely and forcibly bathed by staff. Super old ladies with Heroin addictions, old black guys with delusions of talking to the dead, and this one chick with such a terrifying visage and open mouthed "I'm going to feast on your corpse RAW" look in her eyes it shook me to bits.
Gradually I finally got a rental, got out of there, and now am gainfully employed.

Bottom line, this is the bestthing that could happen to tommy. He just needs the Care and "Little Village" feel of the people in there just like him.

My mom used to work in a hospital with a psych ward, and they can only force this type of thing upon patients with a court order. And the judge won't sign the order unless it's proved to be detrimental to their health. The worst example of this type of thing my mom witnessed there was a homeless guy with a horrific mess of hair. They weren't getting a court order to force it cleaned either, they needed to outright have it shaved, it was that bad. It took a while because the 24-hour med judges deal with medical cases with much higher priority (literal life and death situations) before getting to horrible hair Harry, but yes they did eventually shave his head.

For a 72 hour hold either the staff won't care enough because they deal with similar shit all the time, or they decide a bath is in order but the judge won't see their request before 72 hours is up.

Whenever I went to visit him, I noticed another patient who never changed his clothes and apparently did not want to use the shower facilities there. We both agreed that was nasty as fuck on his part. So, I would assume they usually do not force showers in places like that.
As vain as Tommy is, any nurse worth their salt will be able to trick him into showering with little to no effort. It won’t improve things much, but it’s a good start.
 
Back on August 10th, Tom called the FBI to complain about a YouTube channel calling him a pedophile or something called "Truth". While talking to the FBI agent he said:

I'm totally on edge here, ready to kill anybody who comes through the door if I need to... ...Yeah right, yeah well I had a knife in my hand when the cops answered the door yesterday. But I put it down immediately before I opeed the door.

The FBI then took information from him about the call where the cop came, including the report number.
