Who is he even trying to fool? His supporters would still give him money no matter what he did and everyone else knows he's lying, doesn't care, and are making fun of him for it. Just say, "I'm taking a day off" - the detractors mock him every single day off anyway (because he feels the autistic compulsion to tweet about it every week) so what exactly is he avoiding by whining about an earache?
The only possiblity I can think of is that he's so much of a child that he actually thinks he'll somehow get in trouble if he doesn't provide the same sort of Real Adult Excuse serf nudniks with slave labor jobs have to give when they need to call in.
Exactly no one cares, Phil. You matter so little, as does your stream, that you can do it, or don't do it, and the only person who'll be affected is you. So go for it. Sleep it off or have a getaway with Kat or what-the-fuck-ever. We'll never know what you actually did unless you fuck up and spill the beans and even if you do the most that'll happen is the same people who've been mocking you will continue to do so in a slightly different tone of voice.
