You're going to get a lot of different answers here because health insurance is a huge variable. Medicaid does cover prenatal care and the actual delivery, so if she goes on that (assuming she isn't already) Ralph isn't going to get slammed that hard. Plenty of poor white trash get pregnant all the time and the government covers most or all of it. It's abortions that are the no-no (see Hyde Amendment).
The far bigger expense is after the kid is born. Even if they split, Ralph would likely be on the hook for child support. According to the USDA, the average cost to raise a kid from birth to 17 is now about $234,000. Less in rural areas. Of course, he wouldn't be responsible for all of that, and there's variables there too.
In general, he might actually have a higher out-of-pocket cost for an abortion, but he could be avoiding a far bigger financial albatross around his neck. I don't think he's gonna be bankrupted by just the pregnancy alone. Raising the guntling after it slides out of her fetid birth canal is another matter entirely.

He's fucked, so fucked, but not for the reasons you'd think.
He was premature by two months (and surely born on the taxpayers' dime), his brother has muscular dystrophy if I remember correctly and lives in a home (also on the taxpayers' dime.) A normal birth, just the birth, can cost you as much as $30k in the hospital. You think someone suspected to have Hashimotos is going to have a normal uncomplicated birth? Especially when the father is Ralph, the birthing unit has a serious weed habit and they live in a tar paper shack like animals? That price can easily jump up to $70k or more if they have to do a c-section and NICU support is needed (i.e. baby arrives prematurely.)
But it's not just the birth that gets you, it's the prenatal care appointments. Ralph has bragged in the past about how he'll just go from one ER to the next and never pay the bills because he's a piece of shit - you can't get away with that with prenatal care. They may let you get a payment or two behind but they will cut you off and then you're fucked. You have regular checkups, you have blood work, you have diagnostic tests (here's a really long conclusive list of all the stuff she'll have to do!) He's going to spend a lot of money ferrying her to her different appointments and ubering his mom to dialysis.
Did you know in most states that failure to receive prenatal care is a crime? It is! I've seen some people here say that he'll get her to do a hippy dippy home birth in the bathtub and get a midwife to do the prenatal care, but that won't be free either. The midwife/doula inspects your home to make sure it's clean and safe to have the baby too, no healthcare professional is going to write off on a home/water birth in that shithole.
My friend had three home births and she paid upwards of $30k for two of them (she rented the birthing pool, but that includes all of the diagnostics and pre-natal care too) and the third one she had a hippy birth doula and squatted on a yoga ball. The doula got $5k to just be in the room the bitch didn't do anything other than tell her to keep squatting on the ball.
Ralph doesn't have insurance and she's not his wife anyway, so she's on Medicaid and she'll probably do some good old fashioned benefits fraud to get out of paying for anything. He'll never marry her, he'll just use her as a baby factory to win a petty imaginary slapfight with null and then throw her back to her parents when she ages out. She'll become enraged and like her father will probably create a crazy website detailing the Gunt's crimes and take it to family court to extract all of his lemons.
Ralph will of course poorface for the court when they try to get him for child support, he'll lose his license probably over nonpayment of childsupport and will spend the rest of his life in and out of jail grifting lemons on the internet until he dies.
So basically Ralph's future is destroyed and he did it to himself.
