Some totally responsible medicine being practiced here by Dr. Powers:
This kid obviously has a severely warped perspective:

Hasn't transitioned socially at all? Poor self-esteem? Conscious of social status implications? Also not even gay?
The inhibition towards social transition would obviously be very concerning. After all, how could you know whether you really wanted to leave as a girl if you hadn't even tried? Moreover, why the rigid insistence on the importance of getting HRT first? It doesn't do very much, and you'd end up delaying social transition for possibly years while you waited for it to do something.
Did the clinicians that he sees fail to inform him that Estrogen is a pretty weak hormone and that most of your "transition" comes from learning to use make-up, dress correctly etc. This is especially true for a teenager who isn't anywhere near fully masculinized.
His alt account seems to suggest he has an overly idealized view of what HRT would do:

And this is mildly shocking:

Me thinks he is very deeply immersed in troon-think. And, I suspect he is somewhat less than mature and probably not very intelligent based on the sheer number of solecisms in his posts:

I don't know what "non-euclidean concept of an adult" means. I don't believe the fifth postulate has anything to do with legal & cultural notions of responsibility.
A case of careful what you wish for:
This is the kid's "realization story":

How does he square that with the idea of immutable, innate "transness"? By not thinking about it too much?
I read through his entire post history and couldn't find a straight forward admission of the 'tism but he's obviously of the type. He has:
- ADHD
- In his words "poor verbal communication skills"
- Slightly nerdy obsessions which he talks about in a very autistic sort of way
- He admits to dressing sloppily
- He posts on an ED sub, although he himself is actually fat, so he probably doesn't have an ED.
Interestingly enough, no paraphilic content. I think he just enjoys RPing as a girl on reddit.
I actually didn't know about this, I knew about prostate atrophy (coomers understandably hate it) but I didn't know that it could induce some degree of urinary incontinence:
