The area is constantly exposed to moisture, bodily waste, sweat, pressure, and stress on incisions that makes them prone to popping open and healing improperly. All the handling necessarily for wound cleaning and dilation to keep the stoma open (it's not a vagina ) also lengthens the healing time. Complications such as infection and necrosis are not uncommon. Given his activity level in the past few weeks, combined with all of the other contradicting factors that make him a poor surgical candidate in the first place, I have some serious doubts that these are his pictures.

To expand a bit on point 2, I'd expect to see a picture or video of him sitting on a butt-donut or similar pressure relief device sometime in the last 4 months at the very least.

Edit: Thinking about it a bit more, I'd expect to see a picture of him with a butt donut in public even if he wasn't pictured sitting on it, when you're recovering from major operations in that area you'll be hauling the butt donut around like your life depends on it.
