IIRC if you don't have a gallbladder then the liver takes over that function in addition to it's usual function - so doing something (losing a lot of weight in a short amount of time) that would put strain on the gallbladder ends up putting strain on the liver instead; and given that she already has a fucked liver that is not a good idea. I think that's why she is referencing it, but she goes about it in a very convoluted way and mentioning weight loss surgery just as an example situation where that may occur.Edit: I just noticed in that stream she starts talking about weight loss surgery and the negative effects on the gallbladder. She doesn't have one so why she's talking about it?! Fuck knows!
It's just a convenient excuse for her, as I'm pretty sure the doctor's solution would simply be to try to lose weight and do so with a liver-friendly diet to help mitigate any stress that weightloss may cause. If she was so concerned about her liver then I'm sure she would've been given dietary advice after her gallbladder removal. Advice that she chose to ignore, and ramp up on eating high fat foods and so she ends up with a nearly foot-long (!!!) liver.
