Fatty, if you think your derm needs to ask you about your diet, and get offended when they don't, that means you already know there's potentially something in your diet that is contributing to your acne. And you can google that shit, and apply that information, all by yourself.
When you hurple your 350 or so pounds of excess adipose into a dermatologist's office, and have to pause to catch your breath because the journey between the waiting and exam rooms is so fucking arduous, it's not unreasonable for them to assume that recommending dietary interventions will be a lost cause. You're obviously not interested in changing your diet or giving up anything; the state of your body announces that, loud and clear.
So they do the best you will allow them to do, which means putting you on a drug that's known to have nasty side effects. Your entire body is like a house is on fire, but the only thing you're concerned with is the soot stains it's leaving around the front windows. The derm already knows that any recommendations to extinguish the blaze will be met with resistance and noncompliance, and if you don't care enough to save your own house--well, what are they to do?
Or maybe she wanted to be asked about her diet so she could either get offended by that for internet points, or else tell some bullshit Good Fatty story about how she lives on tiny portions of kale and quinoa but still weighs 500 lbs. Fuck if I know; it's not like fatties are acting from logic.
Your insanity of the day. Remember, if you're thin or trying to lose weight you are not safe for her fat body. Because everything is about THEM.

No, apparently you don't, because if you did, you'd have to accept that many of those reasons for dietary restriction are totally legitimate, and increase or enhance those individuals' well-being.
I like how she says "attempt to control the size of their body"-- as if that's totally futile, and nobody can actually do it, and all of those people are forever locked in a losing battle. Speak for yourself, fatty.
As always, when someone has to tell the world how much love, compassion, and understanding they have for others who they believe to be wrong, they really don't.
