I've had gallstones, really bad ones too, thankfully it's been years so I don't think I'll have to have my gallbladder removed but holy shit it sucks. The pain is directly below your bottom-right rib, right at the end of your ribcage, and it fucking sucks (they never say exactly where their abdominal cramps are, though...I doubt they actually know where women experience menstrual cramps). The first time I had a gallstone attack I thought it was appendicitis and it lasted for no joke more than 12 hours, which is extremely unusual. Normally gallstone attacks are only 30 minutes to an hour, but whatever I haven't had one in years (knock on wood) but it is very cramplike even when it's not severe, and at least for me it did NOT affect my appetite, it was just uncomfortable and painful as hell until it subsided. Gallstones have a large number of potential causes, other common ones for troons (and normal people as well) are obesity and genetics. We all know a lot of troons have really shitty diets in addition to the hormones destroying their bodies. To quote Wikipedia:

High carb diets, low fiber diets can cause gallstones, and those sorts of diets are very common in Western society, especially among terminally online troons who don't know how to take care of themselves. The estrogen probably doesn't help.
