Let's pretend for one moment that he does die before the election, just for the funsies. What happens then? Will the nomination revert to option number 2, aka Bernie Sanders? What Milley did was 100% treason. There is a reason the president is trusted with such a significant chunk of the authorization needed to launch a nuclear strike - because in that event, you really don't want to have a chain of command scenario where you're calling back and forth amongst multiple people to figure out who is actually in charge. So for that reason authorization is traditionally kept compartmentalized but in a very limited way, the president and one other official are what is needed at core.

What Milley did was essentially assure a hostile foreign power that he would be willing to disarm America's highest levels of defense in the name of politics, and he warned Chinese military command of this. This is absolutely treasonous and in any functioning society he would be summarily executed following a hasty court martial seeking to avoid further embarrassment or risk.

The depressing thing is, he won't get punished for it. If anything he'll be rewarded. Military justice does work independent of the doj but the high command is all staffed with party loyalists who would personally piss on the Constitution if they received enough yuan to do it.

Honestly the thing he's going to have to worry about most is getting fragged. Soldiers, unsurprisingly, don't take too kindly to finding out the guy who is supposed to be their goddamn leader in a declared war was just caught selling them out to an enemy they very well may have to face within the coming decades.
Just toss a grenade into the mens room while he's in there taking a shit and the problem is basically solved.

You have great points overall but what of operational or tactical importance is in the cities? The total fertility rate of cities have been negative since ancient times and what's more they still are. They are meat grinders that devour the best of the countryside and produce science, technology, culture and industry. But there isn't much industry close to the cities anymore, the high tech is maybe researched in them but produced elsewhere. In much of the west cities are stomachs and I'm not sure the culture is worth it anymore.
