To be honest I would be considered one of the users who would have been compromised. But frankly I've been hacked before (not on 4chan but elsewhere) and my email was already leaked when internet archive got hacked (and in a strangely similar case of taking advantage of extremely lax security that was never updated)

No matter the website, jannies should never be able to hide behind the scenes, handing out bans and deleting posts for any reason or no reason at all.
Giving a janny anonymity is the absolute worst thing you can, because they're gonna be using this to their advantage when they start malding, shidding and pissing.
There needs to be total transparency and accountability in order to discourage moderators power-tripping so much.
It should never be possible to hand out bans, timeouts or delete posts without the specific janny's name attached to it.

I would also add three day bans with no appeal process is a bad idea. It enables moderator abuse to continue unchecked. At most a un-apealable ban should be 24 hours.
