Years of untreated diabetes combined with alcoholism. Alcohol is already sugary by nature, and Jaegermeister (a liqueur) is sugary. Diabetics have difficulties with healing even if they're keeping their diabetes in check. That's why Blade had the leg holes that would never heal, same deal with his foot-stump not healing now. I would be curious to know what his glucose level is on a blackout night, I wouldn't be surprised if he hit the high 200s, if not mid-300s.

Drinking, especially at Blade's level, is hard on your body even without any pre-existing conditions. I'm honestly shocked that he's lived this long.
