I think people play games dealing with sex and dating as a form of escapism and fantasy fulfillment. By that logic, the characters should look idealized. I mean, if you are a troon that knows that in a million years nobody would mistake you for a woman, then you would like to play like the idealized version of yourself, and date equally atractive people, not be reminded that your fantasies will never come true. If there was an ultra realistic vr dating simulator, would you want to date an avarage person, or someone like Scarlet Johansson or Chris Hemsworth, also, you would like to date them in a fantasy setting, like a luxury boat or something like that. I mean, maybe you could write a VN about the struggles of being transexual, but using a dating simulation template is the worst way to do it. You are trying to inject realism into a genre that exists as a way to fullfill a fantasy.
