I'm not an expert, but aren't you supposed to sew a mounting hole, but leave the Fleshlight free so it can be removed for washing?

Washing stuffed toys is already a dicey prospect. If your kid gets really into one plush toy, smart move is to buy a spare and rotate them each time it has to go in the laundry. This is advice for parents of babies and toddlers, but I guess it'd be the same for neckbeards.

This is interesting, especially for the chest and foot cavities.

I'm aware of silicone breast/butt forms that are designed to add dimensionality to a dakimakura--you put them between the pillow and the pillowcase. Emulating a "tittyfuck" with an onahole, however, is perpendicular to what the angle would be on a three-dimensional catgirl. It seems like it would add sensation at the cost of verisimilitude, essentially simulating a midline chest wound but with no sharp fragments of rib and sternum.
