I know this can be applied to a lot of things, but I can't for the life of me think of what it is if it has a name.
 
"Telephone" or "a game of telephone" is a possible answer for obvious reasons.

There's also "generational decline/decay," in reference to royalty passing their kingdoms down to their inexperienced children.

There is actually a German word for exactly what you are describing, Verschlimmbesserung. However, it doesn't really translate into English. The closest way to describe it in English is 'An attempt to make something better but in the process make it worse instead"
