I have said this before but I think a lot of these people don’t have any meaningful sources of joy or happiness in their lives outside of their sexuality and porn addiction (and this applies even if they do have things that bring normal people joy, like children) and so they assume that other people’s joy or satisfaction, or happiness with themselves, is sexual to some extent as well. Which is why they’ll claim that women have autogynephilia too, because women like getting dressed up and might be happy when they look in the mirror after, and there's no way that doesn't involve getting aroused, right?

With breast reduction surgeries the nipple is removed and grafted back on I think. I also suspect that many women have relatively larger nipples compared to men of the same size so they probably cut them down to a more sensible size for a “man”
