He's probably talking about one of these bad boys. The surgeon sits at a screen with a bunch of controls that send input to the robot, which uses scopes and tools for the surgery. It's supposed to be for really delicate surgery that needs really small motions (the idea is that it can turn big motions into smaller ones with the control interface). It's pretty cool, actually. Has the added advantage of the surgeon not having to stand so close to the amhole.

I've wondered why they don't make the amhole, put something flexible in there to fill the space, and sew the mouth shut, then pop it open a couple months later once the body is used to having a space there. Maybe something that can be expanded over time so it's not agonizing? I suspect genital mangling isn't your area of expertise, but I hope you can answer:

I am not an amhole doctor, unless you count the amhole of the mind. Even still I'm no expert so take everything I say with a grain of salt. That being said, I feel I know more than majority of people in my profession and more about the process and more than most people in general. So here's some healthy guesses. Firstly, even in normal urology complications are normal as that area swells like a motherfucker and is just slow to heal. Genital piercings take about a year for example and that is a very small procedure compared to a new amhole.

This is from a medfag article on what Kevin has:

The vaginal canal can become narrowed and stenotic, resulting in loss of depth and width . Vaginal stenosis is best managed by maintaining serial dilations, although an important minority of patients can still get vaginal stenosis despite diligent dilation. In cases that need an involved revision vaginoplasty, skin grafts may be placed into incisions made into the neovagina. So for re-holing here's the deal, it's kind of a gut job (pun intended):

• Since every surgeon has their own technique I can only throw out specifics, For one he will probably have to have a catheter outside of his pelvis (not from underneath) because of the method they use to suspend it sometimes this is a thing and yes sometimes the catheter is permanent.
• Depending upon the technique (there's options for laparoscopic, I think Kevin wants this one) They have to re-stitch your butthole.... This doesn't take a rocket scientist to see how this might fuck up.
• Still chances of fever, urinary incontinence, fistulas, ganulation, necrosis prolapse, bowel issues, depth loss, development of chrons, This one UK article cites 40% complication rate post-op!

BASICALLY what Kevo is doing is a better option than the Sigmoid colon vaginaplasty from looking at the current literature as the colonplasty gives you anal mucus lining and anus smelling pussy amongst other things. Also the highest level of amhole stenosis seems to come from the penile inversion technique. You lose less depth in this option and don't have to worry about hairballs in your pussy.

As far as I am aware, if it's an eternity collar, there are no keys and it cannot be removed, hence the name. It's only for the most serious of BDSM relationships that are meant to be lifelong. This is one reason I hope Kevin keeps getting fatter forever cuz that's the funniest ending.

Edit: nm I googled it, most have keys to remove it for safety, but yeah it's meant to be worn forever. If you're in a place where it's inappropriate to show your BDSM collar, just wear a higher neckline– but we know that's too much to ask of him.

Looking at the style Kevkev has on, the "key" is honestly just a specialized screwdriver. Most eternity collars that are cheap use a pin screw, actual lock mechanisms are rare and only at the higher end price range, and are generally viewed as a "why would you bother? This shouldn't even need to come off anyways" type thing. The really hardcore eternity collars need a special tool to crimp the metal, making it impossible to remove in a nondestructive manner, or are designed to shear the pin so it can't ever be removed. In the subculture these things are as serious of a commitment and as meaningful, if not even more meaningful, than a wedding ring for showing commitment, but unlike a wedding ring, it has the BDSM flair to it, so that means it's a bit more intense and is designed to be a weighty so you constantly feel what you are wearing, visible, and difficult to remove reminder of commitment. For our cooming Kevin, well, it's a meaningless accessory and part of his LARP.

Since we know there is at least the potential of a key, and this is the tranch, we can assume it is just a pin screw. If we're lucky then they need some weird specialty bit, like a seven pointed star of uneven spacing or something wacky, to remove the pin screw. The far more likely answer though is it is just a Philips, Robinson, blade, or Allen screw. The key is symbolic at best.

Unless a picture is out there of the key or the lock pin/mechanism, I am going to assume it is just a shitty cheap common pin screw.

I know of people who, due to whatever reasons, had a falling out with their dom and the only way to remove the collar was to cut it, and if its too tight that can be rough. It can also cause breathing and eating difficulties. Not only that, but if it is rubbing against you too much it can cause sores and other issues. To prevent such concerns there is something called the two finger rule, basically can you fit in two fingers? If not, don't put it on the sub. Period. If due to whatever reason things change and the rule is no longer true, remove it and replace it.

Since Kevkev is inflating he's likely going to have issues, and since he won't remove it so long as he's with mistrxss complications are likely in his future, and if it needs to be cut off it won't go over nicely. Though considering tranch, he'll likely just be able to get a common screwdriver off amazon to release him.

If you got the right brand, Eternity is a pretty well trusted brand, good quality material, though they aren't the only people who make "eternity collars". Either way, their advertising can be trusted, and until shown otherwise I will assume this is correct until someone provides counter information. Maybe the tranch wasn't as cheap about this as I assumed.

With this new information, well damn he's allowed it to get discoloured and dirtied that much? I shouldn't be surprised, this is the tranch we're talking about. Based on the colouration I thought it was just aluminium that had been against oily skin, I stare at enough metal in my life I am used to how skin contact and air contact affects their appearance, I wouldn't expect that look from even passively cared for stainless steel or titanium, and both are easier to care for than aluminium which oxidizes way easier and gets discoloured more from the oils in your skin, even with good hygiene. I am going to guess Kevkev has the satin stainless steel option, previously I assumed it was a satin or brushed aluminium. The satin also only comes in stainless steel so no need to guess if it is titanium. Makes it easier to cut off at least. Jen seemingly has the black which is only in titanium and Jen's also looks cleaner. This means Jen is the favourite house slave and got an extra $100 spent on his collar.

Regardless of the exact selections made, it uses an Allen screw, and is designed to be easily taken on and off per the site, which is also known to be true. This is actually a desirable trait, as it is important for safety and in the event of needing to go somewhere that such a thing would be inappropriate it can be removed until home. Eternity is also well known for the fact they keep everything really standardized so it is easy to get tools for removal in a pinch if needed. So a suffering Kevkev is unlikely unless he's truly exceptionally incapable of anything and all those around him are equally incompetent. Sadly based on the minimal work they do on the tranch we know at least someone there meets the base line needed to acquire tools and remove the thing from him.
