I'd say they're just highly codependent and have an unhealthy relationship. They probably end up going "poly", since the AGPs invariably seem to want to "explore my evolving sexuality", and the second-transitioner (I'm inclined to say wife, it seems like the husband always transitions first) deals with the alienation by mutilating her own body?
