yeah, there is a transgender ocd theme, aka TOCD. it works a lot like homosexual ocd, aka HOCD.
i'd actually say that is an absolutely textbook case of it, and there's no way it can be anything else.
depends on whether he liked it sexually or was distressed by it or both. but he's clearly mentally ill.
rarely, people with OCD give up because of the torment and act destructively ("checking to see").
e.g. someone who suffers from animal harm OCD for years and kills a rat to "make the thoughts stop".
ofc it only makes the thoughts even worse because there is now "proof". that's what's happening here.
(to not scare away skittish patients, therapists tell people this is not possible. it is possible, just rare.
put yourself into the shoes of someone with decades of this. you'd risk a lot for the chance to end it.)

this is why it's especially dangerous to validate literally anyone having trans thoughts as a trans person.
instead of realizing this was intrusive and getting help, he thought, "wow, the thoughts must be right!"
however, many actual trans people have other OCD themes, because BDD is also obsessive compulsive.
difference between OCD trans and TOCD is the latter transitions only to "fix" the cognitive dissonance.
