If you rewatch he's actually not a good shot and doesn't hit her in the head. The first is a miss. The second one where she puts up her hand caused him to shoot her in the mouth. That's why she started slurring her words. The last two were headshots tho.

Snow-Shovelling Murder-Suicide
(CNN)Three people, including the shooter, are dead after a dispute over snow shoveling in Plains, Pennsylvania, outside Wilkes-Barre on Monday, Plains Township Police Chief Dale Binker told CNN on Tuesday.

At around 9 a.m. Monday, the shooter, Jeffrey Spaide, got into an argument with James and Lisa Goy, who lived across the street, while shoveling snow from his driveway, a release from the Luzerne County District Attorney's Office said.

The Goys were shoveling snow from their parking spots across the street onto Spaide's property, the release said. Spaide asked them to stop and the argument escalated quickly.

As they argued, the release says Spaide went into his house, retrieved a handgun, and came back outside firing multiple shots "at close range" at the Goys. He then went back into his house, pulled out an AR-style rifle, and fired at the injured Goys, "executing them," Binker said.

That second one is awful. I had heard that drowning people don't "look" like they're drowning and this is a perfect example.

Was there something special about the water? I wonder how he got into so much trouble immediately like that.

I grew up near the ocean and this really looks weird. He is doing the motions, not perfectly, but better than a doggy paddle, so he should have been able to move more than he did. Don't think it was cramps, since that usually makes people keel over to a side and move asymetrically. Same with temperatures, you should have been able to see his body freeze up at the cold shock.
My best guess is a whirl forming right at that spot and pulling him back out a bit from the rocks. Just enough that he can't reach it. A real underwater stream would have pulled him in a certain direction and kept moving him that way.
