But that reasonably passing FTM is actually a good illustration about why most don’t achieve that level of passing: this person has a strong browbone, which most adult white males have and which (most, not all) females and younger males do not.

Once you notice that it’s the lack of brow ridge you’re seeing, everything else falls into place:

• Why do younger, beardless FTMs look like 13 year old boys even when they're in their early 20s? Boys around that age haven’t yet developed the full “adult male” bone structure, so your brain interprets their face as younger.
• Why do older FTMs look so… weird? Because they have the other features of adult men (facial hair, weathered skin) while still having that flat upper face that your brain associates with women and children.
• Why do non-whites tend to pass better as the opposite sex? White people tend to have the most sexual dimorphism in the browbone. East Asians of both sexes for example tend to have flatter upper faces (speaking extremely generally here; obviously there are exceptions).

Like the MTF, if you see them on their own without any people around, a few could pass purely on aesthetics. But put them in a lineup of True and Honest Males/Females and it becomes evident when Gimli is hanging out with the menfolk. This person is most likely petite as all hell and barely clears five foot.

Even a genuine short statured man has a kind of bulky architecture underpinning all the externals.

When you say “sunken in” you are probably referring to the eyes being under a protruding brow ridge, as I was talking about in my post. The woman in the picture has an unusually strong brow ridge for a female, which is why she passes to some people.

Keep in mind I am talking averages. Some females have very strong brows (particularly northern Europeans, although that is just my unscientific observation) and some ethnicities have a flatter brow in both sexes.

Why does it seem like the FtM's that get "top" surgery have surgeons that have absolutely no idea where male nipples are supposed to go? Do the people doing these mastectomies not have a plastic surgeon come in and do the final aesthetics and closing?

I have seen similar horror shows with actual women who have had mastectomies related to cancer when it is done by a general surgeon. Some of them the nipples aren't even horizontally aligned. It's scary. Who in their right mind wouldn't have a plastic surgeon do the final aesthetics and closing? You will not only have the best aesthetic outcome, but plastic surgeons are the only surgeons that know the correct tricks in order to minimize/hide scaring. Some plastic surgeons are so good at it, I was amazed to learn the patient has even had a major reconstruction. Without an actual inspection up close, you wouldn't even know they ever had anything done. Some of the talent in plastic surgery is mind blowing. (and some of them would be better off doing horror sfx in Hollywood).
