Basic rule: big weight loss or gain does not play well with cosmetic surgery of really any type. Skin loses elasticity pretty fast after your twenties, which is a major reason why even people who lose comparatively modest amounts of weight after that can be left with saggy skin or tummy aprons. At his size and shape, gaining weight will just round him out more all over. Also, since YWNBAW, his weight gain and loss happens first in his abdomen rather than anywhere near his breasts as happens with actual women. His body is also not built to have essentially two extra bags of skin over where his breasts would be, so they are just stretching chest skin (even allowing for moobs) over two gigantic cyst-like lumps that should not be there. Skin only stretches so much, and it stretches and shears (see stretch marks) along lines and grains in the tissue itself. It doesn't stretch equally in all directions like pulling lycra over something. They wouldn't have taken fat out to put the implants in, because fat removal on the chest wall is a different procedure and basically is paid for and done as a separate procedure. They just shoved the funbags in. The procedure itself is pretty brutal: this lady is pretty well thought of for not manhandling her patients more than absolutely necessary, but this video will give you an idea of even how much pulling and hauling is done by a 'good' surgeon.

This is a helpful basic guide (with drawings, yay) of how breast augmentation is done in the UK. I am not familiar with current US techniques but they are broadly similar. It looks from his result, to me, that they slapped big round implants on top of the chest wall muscle, which gives the worst of the cosmetic results, but is usually cheapest because it's, er, the worst cosmetic result. That 'shelf' effect at the top rules out teardrop shapes and the implant doesn't look well covered enough to be in behind the muscle. The fact he also keeps injecting copium about them "settling" suggests cheap round implants, because teardrops are textured so that they don't "settle" and move around but are more likely to stay where they were fucking put. No one with a thinking brain wants their breasts wandering around their chest wall. If you were an actual woman, this would be a seriously poor result.

Plus, the less well vascularised the area that the nipples are placed on, the worse they are likely to heal and the more likely they are to necrose and perhaps fall off. Moving/reconstructing nipples is difficult work and it gets more difficult the further out they are from major blood vessels. His are just poked out there like Antarctic outposts so those are a world of problems in themselves.

Had a personal experience with a cosmetic surgeon who was shaddy like this (would barely clean his tools since he wants to get people in and out fast enough then go give public lectures on sanitary health), though the more I learn the more I assume it's just all of them.
