I'm going to suggest a fucked up thought here:

We have seen that May was already pretty worried back in October and under the impression shit was bad. We also know he only got worse from there.

On their holiday visit to her family, it wasn't until after Ralph left for Vegas that the Pine Nut poisoning event happened.

Two thoughts occur to me. Either: 1) May intentionally ate the pine nuts to trigger a response in hopes of causing a miscarriage knowing she had family there to take her to a hospital when/if shit goes wrong. Or 2) she actually had an abortion and had time to recover at her mothers before Ralph came home and used the pine nut story as cover.

This explains her depressive look, the belly loss, Ralph unwinding even more, etc. Ralph wouldn't even eat bugs out of fear of it looking like something the a-logs could use. This would have to be certainly kept secret.

Additionally, and this is a bit grim, but I don't think May wanted a baby. I think Ralph did, and she knew she could get her claws into him for the longterm if she got pregnant, and because she's as dumb as a bag of rocks and adapts her personality to whoever she's thrown herself at most recently she did exactly that. Now she's pretty far along, Ralph is having an elongated breakdown due to everything (especially her trying to mother him into eating/drinking better) and keeps absconding leaving her alone with this creature growing in her that she doesn't feel a connection to.
