At my oncology center they have weekly talks from experts. This week's happened to be on a Saturday for once, as the plastic surgeon giving the talk couldn't do the usual Wednesday. Interestingly enough, this surgeon specialises in breast reconstruction for cancer patients. I took the Q&A opportunity to ask this surgeon if they felt there was a good reason for so many of their patients to 'choose' to request no nipples.

He was pretty emphatic that if there was no disease risk or other issue preventing their re-use that it borders on malpractice for a doctor to be encouraging patients so young to be giving up their nipples. He was pretty clearly condemning the entire practice, not just the nipple portion, but wanted to be very circumspect in what he said - which given the current state of politics, I get.

TL;DR: He thinks the nipple tossing is a way to maximize surgical throughput and has nothing to do with improving patient outcomes.

There is significant risk for nipple loss, infection, loss of sensation. Hell, some people have significant range of motion loss for months as the wound heals

A friend of mine lost her boob to cancer and the arm on that side has never been the same. Doesn't move the same way, periodically swells up, lots of problems. I always wonder if the troon kids get that down the line, especially since some of them go on to get the arm penis made.
