Trainspotting is probably the most brutal and realistic portrayal of drug addiction so fair in mainstream media but sadly I'd expect these people to dump all over it because the entirety of the characters are white and extremely poor.

Doesn't Berserk have a sizeable female fanbase? Also, those guys are obviously antagonists. Up until about the early to mid 10's, the average seinen antagonist was like that. Both Guts and Griffith were raped and mutilated, Griffith multiple times actually and it's heavily implied he sold himself as a child to afford food after his parents were killed.
