TL;DR- She is careless in approach/closing and likely has what my medfag friend calls HODAD (Hands of Death and Destruction)
Not a medfag but casual acquaintance with a plastics/reconstructive surgeon and I've listened to him really sperg out about it. Unlike Gallagher though he's usually on-call to make the best out of horrific accidents, not causing the horrific accidents themselves. My laymen's understanding: Incision placement is one of the first considerations. Scarring can't be eliminated but if you can put it in a less noticeable place you're setting yourself up for success. This explains the placement to some degree and it is also why you frequently see similar incisions from augmentations before they mastered laparoscopic techniques. Dr. Plastisperg was also a big fan of closing as much as possible, from as deeply as possible with things like disintegrating sutures. If they're taking up the tension internally it is less likely to cause tension and issues externally.

When you get to finally stitching the incision closed, you must be very careful and precise. In the first one, you're pulling the skin and it's going to want to pull apart. The second one has too much overlap which will cause thickness issues or kelloiding. The third one is in the "Goldilocks" zone: there is not a big overlap but both sides are sufficiently meeting so they do not "want" to pull apart. This guy is pretty autistically/neurotically detail-oriented and it really shows in his work. As far as aftercare he recommends coconut oil lotion after all sutures are out.

But atleast over here in northern europe there has been some pretty big newsreports lately about how fucked up the whole industry is. Its moving slowly....but i think its getting there eventually. Overall i believe people are getting a bit fed up with the whole thing...atleast that is my gut feeling without actually having any more than anecdotal proof of it.
