Doesn't explain why societies with third gender categories didn't have people trying to modify their own bodies, or why societies with third gender categories didn't differentiate between male homosexuality and effeminacy, if there really are that many troon lesbians. It's not just their absence from the west, but the total absence from the historical record of anyone expressing the specific body centered obsessions, that needs to be explained.
