I know that some MTFs will actually get a vaginoplasty made out of a piece of their colon, but I can imagine that something like that is opening the door to complications from the area where the colon segment was taken from, not to mention that this means that the resulting neovagina is literally a second asshole.

@Wernie Sanders These surgeries keep getting infected cos there is shit and piss needing to run through these exact areas, is it a private area kept covered up so can’t have the air that wounds generally need, and it’s in an area that is constantly moving, under pressure, producing sweat and grime.
It’s the worst possible place basically- that and the fact that this field attracts professionals who should really be doing something else if they actually have a fuck about medicine, are in it for the money and are likely less up to standards- lots of waivers are signed before hand so the patient has no rights and this will likely bleed into other areas of practice too, say hygiene...
oh, and it’s private plastic surgery in competition so procedures are jealously guarded, hence there’s no industry standard best practices to reduce any and every complication

The resulting frankenphallus from a phalloplasty also does not have the requisite connective tissues that give a real penis its relative elasticity and durability, so neopenises are also prone to getting microtears in the skin from the attachment site from the weight of it which can get infected or urinary blockages from inflammation which can lead to further infections.
The resulting hole from a vaginoplasty is just that; a ring of scar tissue surrounding what is for all intents and purposes a literal open wound. Unlike a real vagina, there is no ecosystem of beneficial bacteria inside to keep down the population of pathogenic microbes, nor is there the epithelial secretory apparatus to flush out the debris that forms or the surrounding lymphatic tissues to provide immune surveillance.
It is a sad thing when this is stuff that should be pointed out to troons by their physicians looking for a surgical remedy for their delusions

It's the biological click ticking that you can't escape even when your uterus is ripped out. It's quite literally a 'woman thing' that's biologically built into the very cell in your body. A lot, A LOT of tifs seem to start 'properly transitioning' either in their late teens into their early 20's while they spend their teen years pretending at it doing minimal social transition. You don't see half as many of them being older- as you see with nasty late transition Tims. Then they hit their late 20's to early 30's and they'll start feeling the early signs of body telling you you're getting older, you need to settle down, start a family. You're not young and pretty forever. You will be alone and unhappy like you are now if you don't make drastic changes. By your mid 30's that need is being screamed at you in your own mind.
There's also aspects of mental maturity at work + the female habit of stewing negative feelings. As well as a lot of them questioning if they actually feel this way from the very beginning. All of this culminates in the excess amount of female detransitioners you see who are all in roughly the same age range. They wake up one morning and realise they are alone without family, they've fucked up their bodies, and for what? then suddenly it hits them like a ton of bricks and they de-trans in a panic trying to undo what they've done.
This ties into why you see less phalloplasty's as well. There's some utterly wrecking themselves by pursuing that but a large majority of the skyrocketing numbers of girls don't because they don't actually believe their own hype when it comes to being trans. There's a niggling feeling in the back of their minds of what if I'm wrong and then their biological clock shatters itself over their heads while screaming at the them were wrong.
There's gonna be a massive MASSIVE market for breast reconstruction in the future.

Along with the bloodflow problems/glorified armpit that others have said, most of these people are basement dwellers who won't take care of the wound either out of laziness or the disbelief that "it's totally self cleaning guys".
Another thing to consider is asking "why they get infected" is like asking why a woman will bleed out after a coathanger abortion: because the people behind it have no idea what they're doing.

Is she going to opt for the inflatable tube to put in it with the pump in the "scrotum"? I heard that they only last for around ten years or so then they have to be replaced. There is something darkly comedic about having to put an inflationary implant in a makeshift ballsack just to get an erection, and I am not sure if FTMs have much sensation in their "dicks" anyway...I know that MTFs lose most of their nerve endings.

Also if you have a white, liberal, middle class social circle, in your early 20s the only people you see having babies are straight people who used a condom wrong. 7 years ago or even 5 years ago most FTMs were still butch lesbians. There were some who have mild innate bisexuality that got amplified by T making them horny for everything, but overall primary female attracted. This current crop of FTMs has way, way, way more primary or exclusively male attracted young women. Not going to be good. I saw one whining that she was considering detransitioning because she was accustomed to getting lots of male attention and now she isn’t.
