I don't understand why bis skin looks so... lumpy or something. It looks like a permanently trying to heal bumpy rash of some sort. like a bleached orange.

Skin tends to look like that when it's not washed and exfoliated enough. For some people, they wind up basically having both dehydrated (lacking water) and oily skin at the same time. The skin underneath is dehydrated (we all know this nigga ain't drinking water), but the dead skin and excess oils build up on top causing it to look like that. He needs a decent skincare routine, as well as to drink more water, very badly. The dry air where they live likely isn't helping either, but some cleanser and exfoliant would go a long way to help this. Also, I'm pretty sure he had some gnarly acne as a young man, so there's scarring there, too.

I have wondered for a while why troon lower faces look so lumpy.
Most True and Honest wammen I know have a little crease by the side of the mouth where the smile muscles attach, but these fuckers look positively corrugated.
None of my files are working, but I used Penny, Rachel Levine, and Bruce Jenner as examples of this.
Whereas only the most ancient, busted women have that many lines.
Madeleine Albright didn't look that busted in the lower cheek area, and she was a thousand years old.

I might not be as big brained as the trans elder but it sure sounds risky to rush someone's transition. The guy is just getting used to the pool, he might want to take it at his own pace instead of bottom surgery in 3 months. And there's a reason people warn you against taking pills and injections from gray markets.

I'm not 100% sure on this, but women tend to have more fat on their faces than men, which I think is perhaps why men's faces tend to get more "hollow" looking around their mouths as they age.

Actually, men's skin is thicker than womens. Men's skincare products have to be stronger to penetrate deeper.
It could also be other things such as some terfy biological factors

In old pictures he had cystic acne as a teenager, the poor bastard. That's truly unfortunate and he didn't do it to himself so I have some sympathy.

Right, men's dermis and epidermis are thicker, around 25%, and they have larger pores and a rougher texture (all cuz testosterone) but the subcutaneous fat is a different, deeper layer. Women have more subcutaneous fat than men which combines with differing bone structures and causes women to have more heart-shaped and round faces while men's are more square. It might also have to do with men having a wider jawbone because of more robust muscles in the area, so the angle between their cheekbones and jaw makes less of a V-shape, so can look more hollow. (I'm totally guessing on that last part.)
Kevin had laser hair removal on his face, right? If there's not proper aftercare after those treatments, it can lead to scarring as well. If he was as filthy then as he is now, he might have had infections in his beard area that messed up his skin even worse than the acne did. Though, his forehead seems to be the worst, grossest part, probably because he's just a greasy bastard who won't bathe.
