I think it's a lot less (internalized) misogyny and a lot more the desire to constantly be a revolutionary/rebellious/challenge the status quo. In their eyes being a boy and wearing a dress is peak gender rebellion mostly because guys wearing dresses get mocked.
I love how the sexuality and gender flags are separated. Based LGB (LGBA?). I will tolerate the pannies for this.

It's only surprising if you think of it as an identity rather than an exhibitionistic sexual fetish.

My perspective on dealing with Christian transsexuals, although this is not my practical experience as my church simply will not accept transsexuals and has no resource for LGBs either. I recognize that they too are God's creatures, people who have pastoral needs, but this should not be the responsibilities of just any church.
Principles:
1. Set your rubric and make it abundantly clear: tenets of Christianity are absolutely not negotiable. God abhors transsexualism; God abhors worshiping anything apart from Himself (and that include worshiping yourself and your identities -- no one can serve two masters). Your body is the temple of the Holy Spirit and you should use it to glorify God. Your trannsexualism is not a "gift"; God does not make you "the woman you are" (sin did). Eve is not a transsexual.
2. Everyone has his cross to bear; your struggle is not special. Straight people regularly face temptations, and even people who don't think much about sex have to endure trials and temptations from other aspects in life.
3. Your suffering has a spiritual meaning, and you should find a pastor to help you see things in a bigger, closer-to-God picture.
4. This said, God is merciful and understands our weakness and sinfulness only too well. He will forgive every sin if you are honest to him and resolve not to commit the sin again. What's more, you can draw strength from him if only you are willing to.
Practice:
5. If we accept (2), then we can treat transsexualism as just any spiritual stumbling block that people regularly face. I find small fellowship groups, where members take personal concerns after each other, to be ideal. MtF transsexuals should obviously be assigned to an all-male group, for reasons too familiar to repeat here. Still I need to emphasize that men, preferably men from different walks of lives, should be present as the role model of an MtF.
6. In the fellowship group, the men confide with each other about their own problems, problems that they may not share with other people. Other members can offer practical advice but value judgements should be kept minimal (Judgement is for God alone). Building up trust is important for a fellowship group to succeed.
7. Pray. In the group we pray for each other. Prayer is important and should not be dismissed as lip-service. It is a sign that God looks after us and we look after each other.
8. Set tasks and remind each other of tasks. For a transsexual a good task might be this: whenever I have "woman" thoughts, open the Bible (or use an app) and read a random verse and meditate on it. If situation permits, get a small notebook and copy the verse with a pen, note the time. Show your group your notebook and tell them your reflections. Other member might phone the transsexual and ask him about how he is doing.
9. As per (3), find a pastor to discuss your suffering. Sufferings are easier to bear -- and overcome -- if you know of its significance.
10. God does not preclude secular help. If good, non-"affirming" psychological service (in a large church there might be a mental health professional among the flock) is available, avail yourself to it.
