I'm an eurofag and I'm fairly sure I've never, ever seen a bathroom that's not separated by sex. Even at places like campings or beaches where such facilities are small, still separated. The weirdest thing I've seen is bathrooms separated for adults and children when I was little (and haven't seen it again. still separated by sex tho). Even shitty bars or workplaces where the bathroom is an almost literal hole in a wall, you still get 2 holes. Even in construction sites where it's all a sausage fest you get two outhouses. (note: all by law)
