Yes, that's pretty much exactly what it is.

In a way, I am relieved her mesencephalon appears to be totally fucked, because it likely confirms that she cannot experience pain in a meaningful way. I think even a few months ago, the "therapist" grabbing her head like that would have caused a seizure, but she barely reacted in the recent video.

I was interested in Luna’s sleep. Robyn rarely shows that.

We know Luna’s eyes mostly stay open, but it seems the lip smacking and arm movements disappear. I watched her eyes to see if they still went back and forth but on my device the video wasn’t clear and I couldn’t tell. She can’t see so those movements are involuntary. It makes me wonder what sleep is for her? Why does she seemingly get rest. She isn’t dreaming, she’s just there. What part of the brain causes sleep, it’s apparently still there? Or is that the brain stem? I’m sure seizures can be set off in her sleep, but it does seem she can do some form of rest. If Robyn was a normal mom I’d be glad she got that break, but with Robyn, it just gives her time to scam people.

What would occur if the herniation happened? Sudden heart attack, respiratory failure? Would it be quick? (Yes please). Will there be more warnings?

It would be quick, most likely within 48 hours. Initially, she'd probably develop abnormal posturing, which is among the first signs of a central type herniation. She would then likely become completely unconscious, with fixed and dilated pupils. Again, it's difficult to conceptualize how, exactly, any of these symptoms might manifest in Luna because of the gravity of her preexisting brain damage. I'm not even sure, for example, if her pupils are reactive but I would not be surprised if they are not.

Early signs might also include slowing of her heart rate and increase in her blood pressure as her body attempted to maintain blood flow to her brain. Her respirations would become irregular. Initially, she would likely demonstrate Cheyne-Stokes respiration, characterized by a cyclical "crescendo/decrescendo" pattern of rapid, progressively deeper breaths interspersed by progressively lengthening periods of apnea. These three signs - bradycardia, hypertension, and abnormal respirations - are called Cushing's triad or Cushing's reflex and are a physiological response to increased intracranial pressure. It is possible she always exhibits these symptoms to some extent, since even with her shunt I assume her ICP (whoop whoop) is pretty outrageous.

The loss of consciousness is pretty much the point of no return, signaling that so much damage has occurred that the patient will never wake up. If an MRI were to be performed at this stage, it would likely show Duret hemorrhage, an area of bleeding within the pons, which is a very specific sign of central herniation and a grave prognostic indicator. Duret hemorrhage occurs as the pons and lower part of the cerebral cortex are pushed through the tentorial incisura. This causes the branches of the basilar artery, which supplies oxygenated blood to the inferior portions of the brain, including the pons and cerebellum, to be compressed and torn. This finding on an MRI would be considered a portent of imminent death and an indication for withdrawal of aggressive care. She would not be awake or aware at all from these early stages onward.

As the herniation continued, her medulla oblongata, part of the brain stem, would be crushed as the cerebellum was forced through the foramen magnum, the opening in the bottom of her skull where her spinal cord connects to the brain. The medulla oblongata is a vital, extremely ancient structure that controls involuntary functions, such as heart rate, blood pressure, and respiration.

Her breathing would become agonal, the labored, gasping breaths that indicate someone is dying. Within minutes, her breathing would cease completely. Her pulse would become undetectable, though a heart monitor might initially show a rhythm that should produce a palpable pulse, a phenomenon called pulseless electrical activity, although that, too, will soon stop.

And then, Luna will have her hard won and well deserved freedom from the prison she has lived in since the moment of her birth. Robyn will post some videos of herself eye fucking the camera with lengthy and fantastical commentary about how she's still connected with her daughter's cosmic juju and you should buy a Healy or three, as Atlas sticks a fork into an electrical outlet and Glenn browses Grindr in the background. Ashes to ashes, dust to dust.
