Where I work the medicals happen about 50 people a time in the same room so you can easily see what condition everyone is in.
My sample size isn't great but basically everyone that is 35+ has something, be it high blood pressure, high bad cholesterol, fatty liver and so on.
Every year I get told to consult my own doctor about my high blood pressure, every year he says cut out the beer, get some exercise and drink more tea.
There isn't much you can do for being middle aged.

While it's fun to play google doctor and predict that Ralph has one condition that'll kill him within months based on the scary stuff written on webmd I don't think that's realistic.
If Ralph is going to die in the next few years due to getting old/poor lifestyle health conditions it'll be because he has multiple conditions that have been exacerbated by drugs and grog that aren't properly controlled because Ralph is too pigheaded to get a medical and/or listen to the doctor's advice. He might die of heart attack but he'll be in a pretty bad state before that happen.

Personally I think he'll live for a while but his quality of life will be incredibly poor.

While no one can be certain, there are enough physical signs and documentation that offer us more than enough information to come to some strong conclusions.

We know his heart has been weakened from a cardiac episode.

Hes discussed sleep apnea on his own.

His diet and obvious BMI would fit a diabetes profile.
