Reproductive organs are in a special class for transplant so far - you have to give special explicit permission for your uterus to be used for a transplant, it's not covered under regular organ donation guidelines. I imagine that penises would be the same, so even if this started happening there at least wouldn't be a need for guys to worry that it would happen postmortem without their consent. Men's pants are cut differently than women because of the need to make space for the penis/balls. They have a greater curvature at a certain point in the crotch seam. If you get it wrong, you get the rare male camel toe as the pant seam separates the balls and creates overflow. If you've ever watched Project Runway, all of the designers always had issue with menswear because of this fact. They were used to making pants with the sharper curvature of the crotch seam and couldn't get the geometry right when they had to clothe male models and it usually ended up with the models looking like they were wearing diapers.

To be fair, some clothing does have an extra gusset in the crotch, but that's usually ethnic pants like those from India/Pakistan, etc. They are also roomier all over though so it's not just to accommodate the dangly bits.

Super, duper late but if penis transplants ever go mainstream, I'm willing to bet most men would remove themselves from the donor registry. For sane men, the mere thought of losing their penis is terrifying, and I imagine that fear still applies even post mortem. I'm sure every dude can sympathize with other dudes who lost their dicks to injury or disease, but I'm fairly certain the vast majority of penis owners would prefer to be buried/cremated with their penises still firmly attached. A tiny percentage of men might be ok with donating their cocks to fellow men in need of one, but TIFs are absolutely retarded if they think that includes them. Penis transplants have been done successfully before, mainly on cancer victims or soldiers who sustained injuries in combat. Note that few have even been attempted and those that succeed require tons of labor, skill and money. I highly doubt they will go mainstream due to so many barriers. Cookie dough chubs are the best option TIFs have.

The most complex one that has been done to date was this one in 2018 on a veteran through:

It worked surprisingly well but at great cost:
- The procedure was a success but took 14 hours and a 25 surgeon team.
- The cost was estimated to be $300,000 to $400,000.
- The patient was able to pee standing up and get an erection.
- Immunosuppresent drugs will be needed for life as well as a bone marrow transplant from the donor to decrease the risk of rejection.

You are correct about donor scarcity: it took this man 5 years to find someone that was willing to agree to donate their loved ones dick+surrounding tissue and he was a veteran.

Its a fantastic medical procedure for someone thats actually sustained an injury causing them to lose their penis, but I highly doubt any TIFs will be able to fulfill all requirements. And this isn't something that a Thai back ally butcher could pull off in a day even most US based butchers. It's basically and organ transplant with plastics heavily involved. Plus the supporting anatomy is already there. You're just reattaching a penis to a male that used to have one. Not someone who is a female and just decides they want a dick.
