They do that a lot, however I don't think it's (just) because they're pedos. The Japanese are obsessed with cuteness, and that is reflected in Japanese artists' styles. Big eyes, bigger head and smaller body are all characteristics of prepubescent children as you said, and what do most people think children are? Cute.
This has been part of anime/manga 'culture' since it's beginning.
However, how much it affects character design depends on the authors' intent and style, so for series with a "moe" aesthetic, meant to be as cute as possible, they'll go overboard with these features.
Basically, what I'm trying to say is, the series' genre afects their character design a lot. A slice-of-life manga is more likely to have these characteristics than an action anime, but even if they do appear, it could be just to make the audience see a character as cute.
And, yes, they'll also add these to characters meant to be attractive because this 'cuteness culture' is so deeply engrained in Japan that being cute is considered part of the ideal woman over there.
But, obviously, that doesn't excuse them for lolishit or anything like that, I just wanted to inform you of a part of their culture that it seemed you didn't know. You can think this is because they're pedos or whatever other reason comes to mind, I have no idea as well.
TL;DR, the Japs are obsessed with cuteness to an unhealthy degree and that impacts their character design a lot.

It's one thing to find them a bit creepy and I don't blame you, but this is a community majority made up of adults. Unless there's actual evidence of them doing real harm to actual children, then I can't really take you guys seriously. I've yet to have hear about some sort of major pedo ring somehow connected to v-tubers since it became mainstream. Yes, they have their own share of controversies but it has nothing to the stuff you're complaining about. They mostly have to deal with legitimate harassment if they say something about certain countries or if the fans find out they're dating somebody.
I don't have faceblindness if that's what you're implying. I mostly see that applied to real people if they can't remember who they are, but not characters. Maybe if you think certain characters look the same, but that has more so to do with their designs looking similar which can be easily changed depending on who's drawing them.
But you specifically said "women" and not "prepubescent girls" previously. So I thought you meant this in the context of adult women having a youthful face while having a fully mature body. But even then, I don't look at a grown ass woman and think, "she's a child" that's really fucking weird.
I mean adults can be pretty short too and I'm not very tall myself, so it's not an impossibility. Plus the person behind her is an adult too so I don't see how this is an issue.
