I’ve heard of people breaking a tooth although I’m lucky not to have experienced that. But losing half of them before you’re even 40 seems careless?

Powerlevel but my grandma had about 20 teeth left when she passed at age 84, and she was a hard candy eating fiend. She had partials so you couldn't tell.

Some people just get dealt shitty teeth by genes. But I'm not really willing to give a troon the benefit of the doubt when they're all so uniformly gross and disgusting and filthy.
Count the number of alpacas - there are maybe 20-odd in those videos. The scrub is growing in because the herd size is now miniscule. And sure, to give them the benefit of the doubt, they probably have more elsewhere, but.. considering they had over a hundred alpacas alone (not counting other animals), you would really think you would see more of them. I wonder how many alpacas have died because of their neglect or inability to feed and safely house them that they're hiding from the world?
I hate being fair to Penny, but to be fair to Penny, dental care in this country is absolutely abysmal. If you don't have really good dental insurance - which is most of the time separate from your health insurance and thus might not even be provided by your employer at all - you're pretty much screwed because everything dental-related costs an absolute frigging fortune. Even Medicaid and Medicare don't cover much, just cleanings and fillings. The tranch is also in a super rural area, so there might not be any dentists nearby and they certainly lack the means ($$$) to drive hours to go to one, let alone pay out of pocket for the kind of dental care he needs.
That said, it seems like he's probably falling down on the basics - brushing and flossing, that kind of thing, which are exacerbating any other issues that may exist.

I'm not a dentist, but my family has a genetic predisposition towards bad teeth, so I'm very well acquainted with them. In Penny's case I would say it's pretty likely to be carelessness, but genetics, mental illness, disability, malnutrition and other issues can come into play here and can cause serious dental issues even early in life. You can also easily be screwed if, say, your parents pumped you full of soda and didn't give a crap about taking you to the dentist or even making sure you brushed your teeth as a kid. And so on.
(And of course, "trans" kids who have been given so-called "puberty blocker" drugs have major bone issues which includes their teeth! There are reports of adults who were given Lupron as children having severe dental issues, including teeth breaking for seemingly no reason despite otherwise good dental hygiene.)
I know multiple people in my life who ended up with dentures before they hit forty, and there are multiple causes. One had parents who didn't teach them to brush their teeth and never took them to the dentist. Another was in an abusive relationship for years with a man who wouldn't allow her to go to the dentist and limited her freedom, funds, and other means to take care of her teeth. Poor dental hygiene and lack of access to dental care is a huge problem in the US, and people are often undereducated about the importance of dental care. It's sad.
That said... like I said, in Penny's case it very well might be carelessness. Just saying that's not always the case.

I don't know if people just don't realize it or what, but dental insurance isn't super expensive without a group, like medical insurance. My husband and I pay around $50/ month for both of us and they start covering cleanings and fillings on day one, no copay for preventive care. It doesn't cover major stuff (root canal, bridges etc) for 6 months or something, as is the case with most insurance, but the coverage amount grows overtime.
Decay spreads, so most people can prevent ending up in a bad way by brushing at least once a day, getting fairly regular cleanings and taking care of the cavities while they're still small/ isolated. Even without insurance a cleaning and x-rays are probably a few hundred and a single filling is ~$200 (less than $500/ year of done regularly). I know those costs can be prohibitive for really poor people, but Kevin spends almost that much on children's toys in a single month so they could certainly afford it, if it were a priority. Of course, that's not even counting their Medicaid or the tens of thousands in grift money. They just want spend their money on wants and force the taxpayers to cover their needs. Fuck that noise.
