The thing is SHE pointed out to him that there was octopus on the menu because he really likes it, asked him if he wanted it (he did), told him she would order it and then changed her mind but didn't tell Nads she wasn't getting it until the food arrived. Like wtf...why tell him about it, tell him you'll order it and then not order it?! I might have slapped her too under those circumstances.

It could easily be just as simple as her hernia getting bigger and temporally trapping her bowel. That's assuming her self-diagnosed hernia is real, which is quite likely given her vertical incision open abdominal hysterectomy. If she's still capable of clogging the toilet with her numerous poohs, she's shit (pun intended) out of luck getting surgery at her current BMI. She'd need to be actively dying for that to happen in a publically funded health system.
