He has furrows that appear to possibly be surgical scars.

The picture sorta sucks, I'll enhance what I'm talking about:

I'm certain they aren't artifacts. They sorta follow the facial vasculature, but that would be very unusual for someone his age and obesity. I've seen facial features like that on people 70+ that are somewhat thin. Also, notice those darker points along the lines? Those could easily be where surgical monofilament was tied/knotted. I've seen tracts like that on people that underwent facial reconstruction after an auto accident. In auto accidents that involve windshield/glass you can get strangely shaped injuries such as where glass was embedded or sliced the skin full thickness.

What does the rest of the hive mind think?

It does look like some kind of post accident reconstruction the way you point it out.

Fatrick once having been reasonably smart before a serious accident and head injury, but post accident not understanding how much he has changed would 100% explain his behavior and attitude.

But! Surely he would have mentioned being in an accident at some stage?

Could it possibly be the only thing he's ever told the truth about? Especially considering his drinking habits and his own father's booze cruising episode.

But it's also well established that he was a retard back in his school days, so any brain damage that occured didn't really do much to make it worse.
Might be from an accident, I think it's just his fat face and his supremely unfortunate genetics, where all the good parts went to his cousins.

As well as describing him as "an author of science fiction books" (implying they don't believe he's a professional), they also say Fat "says he is being terrorized by online cyber-stalkers" rather than Fat "is being terrorized by online cyber-stalkers".
