Think about it, he's virtually unknown, with diminishing prospects, seeking an audience and affirmation, suddenly he's a massive celebrity with his own unyielding hugebox of fans that tell him he could not be a more perfect being. By normalising the idea that there are women with vaginas and women without through the logic of feminist talking points like patriarchal exclusivity, it's not impossible that you get vunerable, but otherwise astronomically confirmed poofters like this guy who get convinced that they are, actually, physically, obviously, always women. Look, I know the Farms like their spinning Troon wojak, but trannies should be understood as utterly indoctrinated, disregarding the odd grifter. It doesn't even feel like drag though — decently fishy drag is way more feminine and 'real woman'ly for a start, and drag artists in general seem to be parodying something that they're fond of.
Dylan clearly despises actual women, and it seeps through his every pore. There are a lot of arguments to be made about drag culture's relationship to real femininity, but even when it's sending womanhood up it's generally not doing so as maliciously, reductively or spitefully as Dylan does it. Drag might not be everybody's cup of tea, but at least I don't look it in the eye and know in my soul it wants me dead so it can wear my face....
