Edited to add that it looks like our old friend "clitoral" necrosis has claimed another victim. The day 0 photo clearly shows a chunk of bellend intended to resemble a crude approximation of a clitoris. It's gone by day 7 and the area looks awfully black.
While radiotherapy itself is painless, the side effects can be very severe and tend to be cumulative, meaning they get worse as the duration of treatment, and thus the total dose of radiation, increases. Skin burns are an extremely common side effect of radiation and they can be absolutely horrifying. That's contributing to how bad the scars look. Radiation damage also slows wound healing, so the surgical site looks angrier for longer.
So, these SRS butchers are such hacks that the scars look worse than those on women who had an extensive, disfiguring surgery to remove a cancerous tumor, followed by a treatment that burns the skin and underlying tissues, preventing wound healing. As others have mentioned in this thread, cosmetic result is definitely not the first priority of a surgeon performing a mastectomy to remove cancer. Somehow, breast surgeons manage to produce a result that is leaps and bounds less aesthetically offensive than the affirming horrors churned out by the gender hacks, most of whom trained in plastics. I cannot emphasize enough how irregular it is for a board certified plastic surgeon to care so little about wound closure and cosmesis. I honestly find the suggestion that TIFs seek surgery as a form of self-harm and therefore want the most disfiguring, horrific scars to be pretty convincing. It's really quite disturbing.

At least one of the "uglier" scars is on a woman who has very recently received radiation therapy to the tumor bed.
In some cases, adjuvant radiotherapy is given after mastectomy or breast-conserving surgical resection to kill any residual cancer cells and reduce the likelihood of locoregional recurrence (the tumor growing back at the original site). You don't want a tumor to ever recur, of course, but recurrence in the mastectomy scar, ipsilateral breast, chest wall or regional nodal basin is an adverse prognostic indicator, particularly if the cancer comes back very soon after treatment.

Unfortunately for the woman in the photo, it looks like she had a mastectomy prior to the radiotherapy, which probably means she has an aggressive tumor with metastases to at least one regional lymph node.
