My own opinion: thought processes that are "perceived" differently from individual to individual are probably the same thing underneath. A process (like thinking over a decision) might play out essentially the same way, but one person will perceive it as being narrated like they're clearly hearing it, another one will have a fuzzier narration, another one will think it through in images, etc.
It doesn't seem to be a big deal though, since a lot of people with no inner voice never realized they were different until that 2020 viral article when everyone started talking about it.
I my experience it does seem like when it comes to completely batshit crazy NPCs, like Elliot, more often than not you eventually learn they are plural and lack an inner monologue.
We need research on this. Correlation between lack of inner monologue and being an NPC.
I can only contribute anecdotal data but in my limited anecdotal sample group of people I have encountered I would estimate the correlation to be very strong.
