It depends on the condition but at a glance micro penises and enlarged clits do look very similar. When it comes to infants gender is usually determined by urethral location and if there's a vaginal hole.
All genitals start out the same and develop either way in the womb so technically the parts are all there, hormones just decide where to put them. But once puberty hits that's when secondary sex characteristics come in and the addition or lack of hormone causing organs becomes more apparent and can tip the scale one way or the other, so to speak.
TLDR; when they're born probably but I think confusion over adult intersex genitals would mostly be from those who don't know the "extremes" of dick/clit sizing is possible. But if you have more than a passing glance, it's pretty easy to determine if something is a micro dick or a fat clit by reason if there's a vagina or not. It may not be fully developed but it wouldn't just be smooth.
But there isn't a point where intersex makes someone look like a futa fantasy or anything like that, it isn't as ambiguous as people make it sound.
